Community Access for People in Continuing Care Program (CAPCC)

The CAPCC Program was designed to coordinate community access for those individuals who are under the age of 65 and are residing in a continuing care facility.

Eligibility Criteria:

• Be under the age of 65.
• Be residing, for the foreseeable future, in a continuing care facility located in Northwestern Alberta.
• Be socially and/or culturally isolated.
• Be able to personally and actively participate in establishing and following through on their goals.
• Be able to communicate their preferences.
